The shop had no documented medical evaluations for respirator use, no annual fit testing records, and no documented respiratory protection training. The employee routinely wore a half-mask respirator with facial hair that interfered with the face seal and the shop had no policy in place to address it. One afternoon, after exiting the paint booth, the employee experienced a severe coughing and wheezing episode and collapsed on the shop floor. Emergency services were called, and the employee was transported to the hospital and admitted for severe respiratory distress.
